Days & Hours of Work: Monday-Friday (days negotiable) totalling 22.8 hours per week
Competitive Salary: Band 6- $98,867.71 pro-rata + 11.5% superannuation
Location: Council offices in Sandringham and Carnegie, with the flexibility to undertake some occasional work from home

About the Role:

Reporting to the Home Care Packages Access and Intake Team Leader you will provide clients and their carers all relevant information to make an informed decision regarding package acceptance, help them understand their obligations around any applicable Income Tested Fees, and support them to navigate necessary systems including My Aged Care and Services Australia.

You will be responsible for:

  • Responding to enquiries from potential home care package clients and/or their carers living in the Cities of Bayside and Glen Eira.
  • Supporting clients/representatives to understand the benefits of a home care package with Bayside Care and Support taking into consideration their individual circumstances, so they can make an informed decision about accepting a home care package.
  • Complete necessary processes to activate and establish home care packages in readiness for commencement.

Key Selection Criteria:

To be successful in this role, you will bring the following skills and competencies:

  • Tertiary qualification in a health, allied health or social science discipline (minimum bachelor degree level).
  • Minimum two years’ package management experience.
  • Demonstrated experience in conducting community-based assessments.
  • Experience in delivery of person-centred practices and excellent understanding of care management.
  • Demonstrated knowledge and understanding of relevant aged care legislation and community services and supports for the ageing within a consumer directed care approach.
  • Ability to market Bayside Care and Support Home Care Packages as a preferred provider and to influence prospective clients about the benefits of Council’s service.
  • Current Victorian Driver’s licence.

Mandatory pre-employment requirements:

  • Be willing to undergo and complete a satisfactory criminal history check.
  • Provide or obtain an employment-based Working with Children check.
